The type of prescription medication that’s most commonly used in the treatment of erectile dysfunction is phosphodiesterase type 5 inhibitors (including sildenafil, vardenafil and tadalafil). They are used to relax and open the penile tissue, allowing for blood to enter the penis and cause an erection. Intracavernous (base of the penis) injections of vasoactive agents, such as prostaglandin E1, are also used for ED. (32)

Erections are neurovascular events, meaning that nerves and blood vessels (arteries and veins) are involved in the process of an erection and all must work properly to develop a hard erection that lasts long enough. Erection begins with sexual stimulation. Sexual stimulation can be tactile (for example, by a partner touching the penis or by masturbation) or mental (for example, by having sexual fantasies, or viewing porn). Sexual stimulation or sexual arousal causes the nerves going to the penis to release a chemical, nitric oxide. Nitric oxide increases the production of another chemical, cyclic GMP (cGMP), in the muscle of the corpora cavernosa. The cGMP causes the muscles of the corpora cavernosa to relax, and this allows more blood to flow into the penis. The incoming blood fills the corpora cavernosa, making the penis expand.

Erythrocytosis has been noted in men on TRT, and should be monitored every 6–12 months depending upon the patients’ response to changes in haematocrit levels. For mild elevations, the dosage of testosterone can be decreased or the interval of using the medication can be increased. Often known by the common brand name Cialis, tadalafil is a PDE-5 inhibitor that you take once per day to provide 18-36 hours of effectiveness. This significantly reduces the planning typically involved in using other treatments like sildenafil. That said, some users only take tadalafil as needed, and those dosages can sometimes be different. You and your doctor will decide what’s right for you.
The duration of ED depends on what causes it and how you respond to your treatment. Many men notice an immediate improvement when they start on medication or another form of treatment.
